APPOINTMENT:

Child's Certified Birth Certificate

Kindergarten (Blue) Health Assessment if your child's physician has
completed it.

Up-to-Date Immunization Record

Child's Social Security Number (Optional)

Proof of Residence-A bill with Parent/Guardian name and address on the
bill. (telephone, water, etc.)

Affidavit--You will receive in packet, please get notarized and bring
with other documents.
*ENROLLING STUDENTS MUST BE 5 YEARS OLD ON OR BEFORE AUGUST 31, 2012.
STUDENTS MUST REGISTER AT THE SCHOOL IN THE ATTENDANCE AREA IN WHICH YOU
LIVE EVEN IF YOU ARE PLANNING TO REQUEST A REASSIGNMENT.
THE DAY OF REGISTRATION YOU WILL BE NOTIFIED IF YOUR CHILD WILL BE
ATTENDING MOUNT OLIVE OR POPULAR SPRINGS ELEMENTARY IN THE 2012-2013
SCHOOL YEAR. THE DISTRICTS ARE AVAILABLE TO VIEW ON THE COUNTY
